Seeing a specialist without a referral in Sweden
If you have been told you need a referral before anyone will look at your case, that is true of publicly funded specialist care and not true of private care. This page sets out what the private route actually involves — including what you give up, which is the part most descriptions of it leave out.

Private specialist providers in Sweden can see you without a referral. You pay the provider's own fee, which does not count toward the high-cost ceiling, and the healthcare guarantee does not apply because it is a duty on the region for publicly funded care. In exchange you skip the referral step and the public queue. The step to take before you pay
Check whether a publicly funded clinic in another region has a shorter queue than your own. Chapter 9, section 1 of the Patient Act gives you that choice; you follow the receiving region's referral rules and pay the patient fee it charges, and the healthcare guarantee stays with your home region. Queue lengths for the same procedure differ substantially between Swedish regions, and a train journey is often cheaper than a private assessment.
That is not an argument against the private route. It is the comparison worth making consciously, once, before deciding — rather than discovering it afterwards.

Is it legal to see a private specialist without a referral in Sweden?
Yes. Private specialist providers operating outside the regional agreements can see you directly, and no referral is required in the normal case. What changes is who pays: you do, at the provider's own fee, and the visit does not count toward the high-cost ceiling that caps publicly funded patient fees.
What do I give up by going private?
Three things. The subsidised patient fee and the high-cost ceiling. The healthcare guarantee, which is a duty on the region and does not apply to care you arrange yourself. And continuity with the public system, since a private assessment does not automatically enter your regional record or place you in a public queue — you may need to bring the conclusion back yourself.
Will a private second opinion be accepted by my public doctor?
A written assessment from a licensed specialist is a document you can share with any provider, and it will be read. It does not bind the public provider, who reaches their own clinical judgement, and it does not automatically change a decision already made. Its value is that it gives you and your treating doctor a second considered view on the same material.
Is private care faster?
Usually, because there is no referral step and no public queue to enter. But nobody can honestly promise you a date before your case has been looked at, and any provider that promises one before seeing your material is telling you something they cannot know. Speed is the main thing you are buying; treat a guaranteed timeline as a warning sign.
Do I need a personnummer for private care?
Not normally. A private provider can treat you without a Swedish personal identity number, since the fee is not being drawn from a regional budget. Say what your status is when you make contact, because it affects invoicing and any insurance claim you might make afterwards.
Can I claim it on private health insurance?
It depends entirely on your policy, and many employer policies in Sweden require the insurer to authorise care in advance and to direct you to a provider in their network. Check before you book, not after. An assessment you have already paid for is much harder to get reimbursed than one that was approved first.
